Your Employee Assistance Program
EAP stands for Employee Assistance Program. The EAP is a comprehensive problem solving resource that is designed to help you identify personal issues and work to resolve their difficulties. Personal concerns can affect every aspect of your life.
Initial Meeting
A typical first session with the EAP will last about an hour. During this assessment session, the Counselor will work with you to identify your specific areas of concern and outline a plan of action.
Our Counselors
The Performance Plus Counselors have a minimum of five years counseling experience following state licensure in their areas of specialty. These specialties include Social Workers, Licensed Professional Counselors, Psychologists and Marriage and Family Therapists.
Confidentiality
Confidentiality is one of the most important aspects of the program. If you contact the program directly, no one will know about it unless you tell them. Information concerning the nature of your contact will not be released to anyone except upon your written authorization, by court order or as required by law. Participation in the EAP will not jeopardize job security or future promotional opportunities.
Cost
There is no charge for the EAP assessment, referral or follow-up. Fees for the EAP are paid by the Employer. If you are referred to outside agencies and you elect to use those resources, the resulting co-payments and fees, if any, would be your responsibility. Any cost will be kept in mind and discussed when determining a course of action. You will want to check your company's benefit plan for coverage of those services.
Eligibility
Performance Plus services are available for employees and their dependent family members. Adult children who live with you are covered until age 21.

When you just can't work through a situation on your own and need a different perspective, contact your EAP. Here are a few examples where your EAP can be a real help:

Simply contact Performance Plus by calling 1-800-862-3398. Normal work hours are 9:00 AM - 5:00 PM Monday - Friday (Central Time). For non-emergencies, it is best to call during these hours. However, calls are answered 24/7.
A staff member will briefly assess your situation to discuss available alternatives for assistance. Counseling services will take place in a location convenient to your work or home.
